Infirm mum being discharged from hospital into empty house at risk to life Is injunction possible to stop this?

Despite all medical staff at Rotherham District General Hospital saying my infirm mum requires 24 hour care, the very young social worker is overruling them all and sending my infirm, blind mum back to an empty house with poor care package. Is an injunction to stop this happening possible? I'm in receipt of pension credit, will I get help with costs? Will this social worker be responsible should anything happen to mum?
